Interstate 181 runs northwest through the tri-city area of northeast Tennessee, overlapping with US 23. I-181 north on U.S. 23. 321/SR 67 near Roan Hill in Johnson City. Between downtown and Tannery Knobs, the freeway headed north through town. Indian Region Crossing I-181/U.S. 23 continues northwest to Gray and Interstate 81 in the unincorporated community of Sullivan County.

I-181 North travels in a rural direction between Birdwell and Buckalo Ridges into the Rock Springs Valley. Running between Easley Ridge and Pine Hill, I-181/U.S. 23 enters south from Kingsport. Continuing west of the city, I-181 crosses the creek and the Holston River Hills to the east, entering a wide valley fed by the Holston River in South Florida. The US 23 Interstate portion ends at an interchange with US 11W in Fort Robinson north of Kingsport.
The freeway at U.S. 23 and eventually Interstate 181 was built in stages in the 1970s and 1980s as part of the Appalachian Development Corridor B construction project. The route between SR 67 in Johnson City and I-81 connects to the interstate system at USC. 139 (a) on 4 December 1985 I-181 is a 23.8-mile long U.S. highway designated by the American Association of State Highway and Transportation Officials (AASHTO). June 10, 1986 in Section 23: Section B of this APD Corridor. Practically classified as a major highway and recently fully controlled, the highway has been built equal to interstate standards. The route is a logical addition to the system that allows direct interstate routes to I-81 from cities with more than 50,000 residents.

A separate proposal also described renumbering I-181 north of I-26/81 as the new Interstate 126. Rejected June 2003. Interstate 26 was opened on August 5, 2003, and the portion of Interstate 181 south of I-81 was eliminated. . The Interstate 181 signs were completely removed in March 2007, and the remaining miles were incorporated into Interstate 26.

I-181 was shortened to 8.40 miles before being completely removed. This number is reported in the Interstate Road Log & Finder list as of December 31, 2018.
The highway connecting Johnson City and Kingsport in the Tri-Cities area was originally named Tennessee State Route 137. Going north at Exit 4, it is now SR 126.
The freeway at SR 137 extends south from the US. 11E (Roan Street) to Market Street in Johnson City Center from 1973 to 1976. US 19W/23 was transferred to the highway in 1978.
