Press Control-N to open a new document window. Go to the print section and select the A4 template. This automatically sets the width to 210 millimeters and the height to 297 millimeters. Increase both values ​​to 6px, indicating the bleed area.

Now that the width is set to 216mm and the height is 303mm, make sure the resolution is set to 300px and since you’re creating it for printing, choose CMYK for the color mode. Set the background to transparent and then click the Create button to create your new document.

Go to Edit > Options > Units and Metrics and make sure Metrics is set to Millimeters. Now that your document is created, let’s learn how to create a nanny form.

To enable Smart Guides, go to: View > View > Smart Guides. Now you can control-drag over all the elements to check the spacing between those elements and your new text.

Select the Rectangle Tool (U) again and use it to create a 1260 x 280 pixel shape. Set the fill color of this rectangle to C=66% M=0% Y=41% K=0% and place it as shown in the first image.

Switch to the Horizontal Type Tool (T) and return to the Character panel to increase the size to 50 and decrease the tracking to 20. Type “LISA JOHANSSON” and place this new text in your new rectangle.

Hold down the Alt key to easily move a copy of this text, then hold down the Shift key to constrain the copy’s movement vertically. Place it as shown in the image below and replace the text with the text shown below.

Select the Rectangle Tool (U) again and use it to create a 900 x 200 px shape. Place it as shown in the following image and then focus on the control panel to style it.

First, remove the fill color and set the stroke color to white. Increase the Stroke Weight to 20px and then click the Stroke Options button where you want to set Alignment Out.

Select the Polygon Tool (U) from your toolbar and click on the canvas to create a new shape.

Set the Width and Height to 670px, enter 8 sides, increase the Corner Radius to 50px and the Start Aspect Ratio to 90%, then click OK to create your new shape. Fill it with C=0% M=90% Y=45% K=0% and position it roughly as shown in the following image.
